ICO reprimands University Hospitals of Derby and Burton NHS Foundation Trust

The University Hospitals of Derby and Burton NHS Foundation Trust (UHDB) has been reprimanded by the Information Commissioner’s Office (ICO) due to a computer system error that caused delays or loss of patient referrals. The investigation found UHDB failed to implement a formal process or apply a suitable level of security for processing referrals. The error affected nearly 5,000 patients, of which more than 4,100 experienced delays causing distress and inconvenience, while 569 referrals disappeared from the system altogether. The reprimand recommends supporting affected patients, implementing preventive measures, and sharing learning from the mistakes across the Trust.
