ICO reprimands South Tees Hospitals NHS Foundation Trust

South Tees Hospitals NHS Foundation Trust has been reprimanded by the Information Commissioner’s Office (ICO) for a data breach that led to the disclosure of sensitive information to an unauthorized family member. The breach occurred in November 2022 when a Trust employee sent a standard letter to inform a patient's father of an upcoming appointment, but the letter was sent to the wrong address. The subsequent investigation by the ICO found that the breach was caused by human error and that the Trust had not fully prepared its staff to handle sensitive correspondence appropriately.
