Yorkshire Ambulance Service NHS Trust has launched an investigation after two staff members allegedly accessed or attempted to access the medical and incident records of William Chapman without a legitimate reason.

The potential data breach occurred following an incident at Yorkshire Wildlife Park, where 23-year-old Chapman died after entering a tiger enclosure. The trust identified the unauthorised access through internal audit controls and subsequently notified both the deceased's family and the Information Commission (ICO). The ambulance service said it maintains robust safeguards and takes patient confidentiality very seriously.
